Class I and II Medical Form
This form is required annually for all youth and adults under 40 attending council-sponsored camping programs lasting longer than 48 hours. It is required for summer camp.
Class III Medical Form
This form is required annually for all adults over 40 and all youth participating in high adventure activities.
